What is sleep apnea?

OSA (obstructive sleep apnea) is a condition in which an individual stops breathing while sleeping; there can be hundreds of these episodes per night. These pauses in breathing are known as apneas or apneic events. The word apnea (Gk, a – “absence of” + pnoē – “breathing”) literally means absence of breathing. Absence of airflow for 10 seconds or longer is considered an apneic event.

An apneic event occurs as follows:

  • Air passes through the upper airway (mouth, nose, and throat) on its path to the lungs
  • Normally, the soft tissue in the throat collapses inward during a breath
  • Dilator muscles widen this gap, keeping the airway open by working against the collapse. Abnormalities or interference cause turbulence in the airflow
  • If the throat tissue collapses, and the airway becomes blocked momentarily; an apneic event occurs and breathing pauses
  • The lack of oxygen causes the brain to send out distress signals, and the sleeper, although unaware of it, partially wakens and resumes breathing. This is typically accompanied by sounds of gasping or snorting

Symptoms and causes

The common symptoms of this disorder include morning headaches and dry mouth, excessive daytime fatigue or sleepiness, irritability, depression, impaired mental function, and loud interrupted snoring. OSA can be caused by a number of conditions including:

  • Weakness or abnormalities in the muscles surrounding the airway
  • Abnormalities in the structure of the airway, face, or skull
  • Enlarged tongue or tonsils
  • Conditions such as acromegaly, retrognathia, or micrognathia
  • Gender, age, ethnicity, family history, obesity, physical characteristics, medical conditions, smoking, and alcohol use also play a role in the occurrence of OSA

A CPAP (Continuous Positive Airflow Pressure) device is commonly prescribed to treat moderate to severe obstructive sleep apnea. This treatment requires the patient to wear a nose mask that delivers a steady stream of air to prevent the airway from collapsing. Although CPAP is effective, patient compliance is low due to discomfort and inconvenience.

I've been going to Dr. Arellano for about 20 years and I continue because of the excellent care and treatment I get with him and his whole office team. He is both professional and personable.
Recently I had a newer technique of gum surgery/grafting at his office called, "Pinhole Surgical Technique of gum grafting." I personally found this to be much easier to handle than the other style of gum grafting that I received years ago for the other side of my mouth. That worked, it was where part of my roof of my mouth was cut and surgically sewn onto the gum. However, I much prefer the pinhole technique. It was actually the easiest dental work I've had (beyond regular cleaning.) It didn't take too long and it didn't feel uncomfortable getting it done. I was swollen, but I think that happens regardless of what kind of gum grafting is done. I needed ibuprofin for a few days, no major pain. It healed well and just felt easy. I am not a fan of having any work done in my mouth because that's me, but this was the easiest work I've ever had done.
So, if you are debating which kind of surgery to do I personally would recommend the pinhole surgery.
